3rd Annual Walk for the Dogs

Pre-register for Dogs4Diabetics' 3rd Annual Walk for the Dogs below. After completing this form you will be asked to submit your $20 registration fee by completing D4D's donation form.

An expedited sign-in table will be available on the day of the walk for those who register, and all pre-registered participants will receive a free event t-shirt. Please remember to print out the Sponsorship and Liability Release forms and bring them with you, along with your collected donations, to walk registration on the morning of August 21, 2010.
